Northwest Transitions – Adult Group Home

Northwest Transitions is a program within Northwest Passage and is licensed by the state of Wisconsin as a four bed adult family home. The group home is contracted with St. Croix County Mental Health and is working as part of their community support program and emergency services to provide services for adults who are classified by the State of Wisconsin as having emotional disturbances/mental illness (MH).

The ranch style home is located in New Richmond, Wisconsin with four single occupancy bedrooms, two kitchens, large dining and living room areas, recreational room, and good access to both the community and nature. There is a main floor laundry, a spacious kitchen and a large outdoor patio. Northwest Transitions provides a home environment by accommodating personal tastes and preferences in décor and house activities.

The program…

The program is focused on meeting each consumer’s specific needs, where each consumer has an individual service plan or recovery plan that is tailored to help stabilize and direct the appropriate care necessary to leave the facility with the ability to better integrate back into their community.

Examples of the services offered at the home include assistance and training with activities of daily living, medication monitoring, social and recreational skill training, employment skills training, transportation services, recreational events, and individual therapeutic support.

The caring staff at Northwest Transitions provides mentorship and spends time connecting with the consumer, helping them make positive gains in socialization, recreation, health, vocation, and overall quality of life. We encourage independence and empowerment, giving the consumer positive choices throughout their stay. The goal at Northwest Transitions is to provide a home-like crisis stabilization within the community that promotes skill building, independence and a consumer-centered focus.
